Common Rejection Reasons
The uploaded document is not an errors & omissions policy.
Explanation: The uploaded document may be a receipt for your policy, a rider, or other documents that is not an actual E&O policy.
How to solve it: Please upload the entire E&O policy, with the page that contains a signature by the insurer or authorized representative.
The uploaded document is a bond, not an errors & omissions policy.
Explanation: The uploaded document is a notary bond, which is different from an errors & omissions policy.
How to solve it: Please upload the entire E&O policy, with the page that contains a signature by the insurer or authorized representative.
Incomplete document.
Explanation: We need to see the full E&O policy in order to verify that it is valid. We often receive an incomplete policy with just one page of the document or multi-page policies split between multiple uploaded policies. Our verification system is unable to process a multi-page document that is split across files and uploaded separately.
How to solve it: Please upload the entire E&O policy, including the page that has a signature by the insurer or authorized representative, as one policy. If your E&O is split between multiple files, please follow our uploading instructions here so that our system can automatically merge those files into one for you upon uploading.
Incomplete document. Please upload the full policy, with the insurer's signature.
Explanation: We need to see the full E&O policy in order to verify that it is valid. Many times, we receive an incomplete document where only one page of the entire policy has been uploaded. The rest of the policy usually contains a signature by the insurer or authorized representative, which we look for.
How to solve it: Please upload the entire E&O policy, which contains a signature by the insurer or authorized representative. If you're not sure whether the copy of your policy is complete, we recommend contacting your provider to make sure. If your policy contains multiple pages and is spread across multiple files, our system can automatically merge those files into one for you - just follow the uploading instructions here.
The name on this document doesn't match [your name]. Please add your company name to your profile.
Explanation: We need to see that the name on the E&O policy matches the name on your Snapdocs profile. In this case, your business was the name insured on the policy, but there was no company name on your Snapdocs profile, so we couldn't find a match.
How to solve it: Please add your company name to your Snapdocs profile and then re-upload your E&O. You can add your company name by going to "My Profile" and then adding your business name in the "Company name" field.
Important Note: If you have changed your name, please scan a legal proof of name change with the document.
The name on this document doesn't match [insert name].
Explanation: The verification system looks for a match between the name on your Snapdocs profile and the name on your credentials. If there's not a likely match, we're unable to verify your E&O policy.
How to solve it: We recommend using the name on your documents as the name on your Snapdocs profile to ensure that your documents can be easily verified by our system. You can change the name on your Snapdocs profile by logging in and going to "My Profile" or by going to https://app.snapdocs.com/profile/edit. Once you've updated the name on your profile to match the name on your documents, just re-upload your documents so our system can take another look.
Illegible document. Please upload a clearer copy.
Explanation: The uploaded document could not be read by the system, so it could not be successfully verified.
How to solve it: Please take another picture or scan of your E&O policy and ensure that the information on it (your name, commission number, expiration date, etc.) can be clearly read. Please also make sure that the entire policy is visible and nothing is cut off. Then, upload your policy again to have it verified.

Explanation: Part of the document has been cut off, which means that our system couldn't verify the information on your document.
How to solve it: Please take another picture or scan of your E&O policy and ensure that the entire document or page is visible in the image. Please also make sure that the information on your policy can be clearly read. Then, upload your policy again to have it verified.
The document could not be opened. Please upload a PDF, JPG/JPEG, or PNG file.
Explanation: In order to view and verify your E&O policy, our system must be able to open and access the uploaded file. Any other file formats, besides a PDF, JPG/JPEG, or PNG file, cannot be opened by our system and therefore, cannot be verified. File formats that we cannot verify include:

How to solve it: Please convert your file into either a PDF, JPG/JPEG, or PNG file and then upload it to have it verified. If you're not sure how to convert your current file, we suggest re-scanning your policy or taking a picture of your policy using either a camera or phone.
